Registration Information Sheet

Two Ways to Register:
1. All players may register online
2.. Turn in all paperwork at an official registration
     Here is what you will need:

        (1) REGISTRATION FORM Completed

        (2) Cash or Check for registration fees

            (U10 & Above-$55 – 1st Child, $50 - 2nd Child) (U6/U8-$55 - for 1st child & includes t-shirt, $50 for

                    2nd child & includes a t-shirt)

        (3) Wallet size photo for each child over the age of 10

                  (teams of U12 and above)

        (4) A photo copy of your child Birth Certificate

                  (New players must have a photo copy of their birth certificate

                    attached to the Registration form.)

players Eligibility:

        Parent or guardian must complete and sign the registration form for each child. Players

       MUST BE at least 4 years old to register - NO   EXCEPTIONS. Player must be 4 by

        August 1st to play in the Fall season & February 1st to play in the spring season, or

        he/she will not be eligible to play.



Team formation guidelines:

        Returning rights for players on teams in the Spring season are only guaranteed through

        the end of the second official registration. New players are placed on requested teams

        by the age group coordinators based on first come - first served. There are NO guarantees.

        Players are placed on teams of their own age group based on their age as of August 1.

        Teams are all girls, all boys, and co-ed (by request).

        Please mark at the top of the registration form which team or coach is preferred.



Coach, assistant coach guidelines:

        Parents wishing to volunteer to coach or be an assistant coach can place a check

        mark on the player registration form under PARENTAL SUPPORT.

        Only the head coach can identify an assistant coach to help with a team. This will

        result in the children of the coach and assistant coach being placed on the same team.

        The head coach can identify only one assistant coach.

        The team coordinator will call if the team needs a coach or assistant coach.





IF A REGISTRATION FORM IS INCOMPLETE, IT WILL BE RETURNED. AN ACCOMPANYING CHECK SHEET WILL INDICATE WHAT INFORMATION IS INCORRECT OR MISSING. THE PLAYER IS NOT REGISTERED UNTIL ALL OF THE REGISTRATION MATERIALS ARE TURNED IN TO THE REGISTRAR.
